Description

Introduction. This is a non-personal services contract to provide Elevator and Chairlift Preven-tive Maintenance (PM), repair, test, inspection and certification services. The Government will not ex-ercise any supervision or control over the contract service providers. The Contractor shall perform to the standards specified in this Performance Work Statement (PWS). Contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the Government. The Contrac-tor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervi-sion, and other items and non-personal services necessary as defined in this PWS. The Contractor is responsible for providing all parts required for Preventive Maintenance Orders (PMOs) and all repair parts required for Demand Maintenance Orders (DMOs). Background. The United States Army Garrison Hawaii (USAG Hawaii) requires preventive maintenance, repairs, testing, inspection and certification services for elevators and chairlifts at Army installation on the Island of Oahu, Hawaii. Technical Exhibits (TE) 1 through 6 lists the current inven-tory of elevators and chairlifts for this requirement. The number of elevators and chairlifts may in-crease or decrease, the Government may negotiate changes and modify the contract accordingly. Objectives. The Contractor shall ensure all work accomplished in the performance of this PWS meets all applicable Federal, State, and Local Laws, Regulations and Directives to include, but not limited to, the publications in Section 7, Related Documents. The Contractor shall provide all neces-sary labor, management, supervision, materials, tools, equipment, supplies and transportation neces-sary to perform the services as specified herein this Performance Work Statement (PWS). A complete quote must be received to be considered for award. Quotes that fail to address all requirements or follow all instructions may not be considered eligible for contract award. A complete quote includes a response and submission to each of the following: 1. General Information: Offeror Business Name, Address, CAGE Code, and UEI number (ensure Representation and Certifications are up to date in SAM.gov). Include Business Size and Type of Small Business based on the applicable NAICS Code, Federal Tax ID, Primary Point of Contact (to include telephone and e-mail address), and payment terms (i.e. net30). 2. Technical Acceptability Submission: The Offeror shall submit, as part of its quote, a technical acceptability narrative not to exceed 10 pages with 12-point font, demonstrating the Offeror's thorough understanding of the scope of work and capability to perform the services outlined in the Performance Work Statement (PWS). The Technical Acceptability Submission should be a single, stand-alone document containing all information necessary for the Government to perform a complete technical evaluation. To facilitate evaluation, this document must be formatted to clearly address each of the technical acceptability subfactors outlined in 52.212-2 and the added addendum to 52.212-2.
