Amendment 4 Load updated PWS and Price Schedule from Amendment 2 Amendment 3 The purpose of this amendment is to answer 5 additional proposed questions (see attachment 12), and extend the solicitation for two weeks until July 24, 2026 @ 3:00 P.M. EST. Amendment 2 The purpose of this amendment is to answer 39 additional proposed questions (see attachment 11), update attachment 1 PWS, the price schedule attachment 2, and attachment 5 Addenda to 52.212?1 Instructions to offerors, and add attachment 10. PWS Changes New pickup locations, up to a 50-mile radius, may be unilaterally added to the contract by the USG at any time during the life of the contract at no additional charges to the USG. The Autumnal Equinox Day date updated to September 23. Removed "and approved by Disposition Services" from PWS 2.2.3 PWS 2.6.1(d) CLIN RE08: Mercury, High, Dental Amalgams, Debris & Residues, Hg >260 mg/Kg. Removed 1500 GL minimum requirement. Removed verbiage in the PWS that references minimum TO requirements. Removed CLIN 1093SV Surcharge, Less Than Minimum Task Order. Price schedule Changes In the price schedule, it states "see 3.20", however there is no 3.20 in the PWS. Attachment 5Addenda to 52.212?1 Instructions to offerors “Completed DLA Disposition Services – Contractor Representative (PWS Section 6.4).” changed to PWS Section 6.3. The purpose of this amendment is to answer proposed questions. 1. Site Visit Will a site visit be held for this solicitation? Attachment 4 includes DLAD provision L07 (Site Visit Instructions), but the contact fields are blank. If a site visit is available, please advise the date and registration procedure. PCBs The synopsis lists PCBs in the scope of work, but we could not find PCB-specific CLINs in Attachment 2 (Price Schedule). Please confirm whether PCB wastes are covered under this contract and, if so, which CLINs apply. The contract states on page 10. Section 1.1.1 CONTRACT LINE-ITEM NUMBER (CLIN) SELECTION CRITERIA (1) PCB-Related, Containing or Contaminated with - Not applicable to this Contract. 3. Qualified Facilities List (QFL) May we submit an offer based on a TSDF that is not yet on the QFL? We plan to pursue QFL registration (DLA Form 2507) for the facility in parallel with the proposal process. Yes, however, will have to go through vetting to add any new TSDFs. (Section 5.1) PWS. 4. Question Cutoff Date Please confirm the exact cutoff date for offeror questions. June 23, 2026 12:00EST 5. The Qualified Facilities List published on the DLA Disposition Services website to identify disposal facilities for PFAS waste. We found that the PFAS Disposal List on that page states that the facilities named there are the sole authorized recipients for the final disposal of PFAS waste, and all of those facilities are located in the United States. On the other hand, the PWS states that all waste under this contract shall be disposed of or recycled only at DLA-approved TSDFs located in the Government of Japan (GOJ). The two documents appear to point in different directions for PFAS waste. For CLINs PF20, PF21, and PF22 under this contract, please confirm: 6. May PFAS waste be incinerated at a GOJ-licensed facility in Japan as final disposal, subject to QFL approval of that facility? Yes, PWS- Section 2.4 Base CLINs PF20, PF21, and PF22 for incineration.
