Description
The Department of Health and Human Services (HHS), Centers for Disease Control (CDC) plans to procure, on a sole source basis, integration of simulation models into a unified backend platform from: RTI International P.O. Box 12194 Research Triangle Park, NC 27709-2194 The closing date of this notice is in accordance with FAR 5.203(b). This acquisition will be processed under FAR Part 12 – Acquisition for Commercial Items and will be made pursuant to the authority in FAR Part 13.106-1 (b)(1) and is exempt from the requirements of FAR Part 6. The North American Industry Classification System code (NAICS) is 513210 - Software Publishers, and the small business size standard is $47M. The purpose of this project is to design, develop, validate, document, and deploy a unified backend platform integrating the CDS Model and SDOH Model while preserving validated functionality and maintaining separate user interfaces. The National Center for Chronic Disease Prevention and Health Promotion (NCCDPHP), Division of Diabetes Translation (DDT), develops and maintains microsimulation models to estimate the long-term health and economic outcomes of diabetes prevention and management interventions. These models inform federal policy, program planning, and evaluation. Since 2016, DDT has developed two Python-based microsimulation platforms: Comprehensive Diabetes Simulation (CDS) Model. The CDS Model includes separate Type 1 and Type 2 diabetes microsimulation models. These models simulate disease progression from prediabetes or diabetes diagnosis through death. They estimate long-term clinical, economic, and quality-of-life outcomes associated with interventions. The CDS Model supports cost-effectiveness analyses of individual-level interventions. Users may modify intervention parameters and define population characteristics. The model has been deployed on CDC’s internal website since 2020 and is actively used to support DDT’s mission and conduct applied epidemiologic health economics research. The CDS Model is transitioning from a development environment to a production environment within CDC’s network. Social Determinants of Health (SDOH) Diabetes Model. The SDOH Model is a microsimulation model designed to evaluate community-level policies, programs, and practices (PPPs) that influence risk factors for diabetes and its complications across five NCCDPHP SDOH priority domains. The SDOH Model estimates the long-term cost-effectiveness of community-level interventions affecting Type 2 diabetes incidence, complications, mortality, health-related quality of life, and healthcare costs. It uses CDS Model disease progression and complication equations as part of its computational engine. The SDOH Model is programmed in Python and allows users to modify intervention parameters and define target populations. It is deployed in CDC’s internal development environment but has not yet been released to production. Although the CDS Model and SDOH Model share core disease progression logic, they currently operate under: •Separate backend codebases •Separate repositories •Independent security scanning pipelines •Distinct deployment workflows This duplication increases maintenance burden, regression testing requirements, DevSecOps complexity, security review workload, and risk of computational inconsistencies. CDC requires integration of both models into a single unified backend architecture while maintaining separate user interfaces accessible via distinct URLs. RTI International developed the two simulation models and possesses the necessary expertise to complete the tasks required for this project, including epidemiologists, statisticians, and computer programmers. As the original developer, RTI has in-depth knowledge of the design, data, methods, and programming codes used in these models.
